Client Forms

On your first visit with me I will ask you to complete a Client Health History form or you may print the form, complete and bring with you to your session. The other forms are optional. It's advisable to arrive a few minutes before your scheduled start time in order to fill out forms and/or discuss with your therapist what issues need to be addressed so as not to cut into your massage time.
